How do add an image to a survey?

You may want to add an image or a picture to your survey.  To do this, you can use the "Instructions or Information" question type which will allow you to add HTML your survey.

First, choose the "Instructions or Information" question type.

Next select the picture icon.  

You can upload your image for us to host your can put a URL for where you image is on the internet.

Then your image will appear in your survey wherever the "Instructions or Information" question has been inserted.
